Body shape can be adjusted but the shoulders which are the main structure can not be changed.
It depends on the fit, but yes it can. The cost also depends on which part needs to be brought in and by how much. It is always suggested to go for the nearest correct size but I understand that for whatever reason, that might not be possible. Our sizes go up in 2 inch increments so you are looking at 1.5 size reduction. It's very doable but will need to be assesed first to see what needs adjusting.
Altering a size 42 suit down to a 39 is quite a significant adjustment. While minor alterations (like taking in the waist or adjusting sleeve length) are common, reducing a suit by three full sizes can be tricky. It may affect the overall fit, proportions, and structure of the jacket and trousers.
A skilled tailor might be able to achieve it, but at that point, it’s often better to start with the right size for a cleaner, more natural fit.
